DOCUMENT CONTENT

  1. External, non-campus entities looking to utilize Department of Athletics & Recreation facilities located at the State University of New York College at Fredonia must have the following on file in order to be considered for approval:
    1. A Revocable Permit, which requires the signature of a Notary Public, is available upon request by emailing athletics@fredonia.edu
    1. The notarized permit must be accompanied by a Certificate of Insurance, naming SUNY Fredonia as additional insured for $2million.
    1. These are non-negotiable, and MUST be completed before you can reserve space.
    1. Revocable Permit
    2. Certificate of Insurance
    3. Both documents will be considered valid for one year from the date it is generated.
    4. Rental Rates may apply.
    5. For events involving children 17-years of age and younger, there is mandatory paperwork pertaining to the SUNY and Fredonia Child Protection Policy.
    6. Requests must be received at a minimum one-month in advance.
  2. Internal, on-campus areas looking to utilize Department of Athletics & Recreation facilities located at the State University of New York College at Fredonia must have the following on file in order to be considered for approval:

    1. Sponsoring on-campus office.
    2. For student groups, verification of Student Association status.
    3. Rental Rates may apply.
