VoIP Phone Location Change Form

The Desk phone location change form found at the following link is for University employees only.


It is required to submit this form when moving a desk phone from one location to another, whether within the same building or to another building. The completion of this form is required to ensure that the University complies with 911 regulatory requirements.

Requests must be submitted no less than 3 business days from the requested date of phone relocation.

NOTE: This form is for approvals to change the location of your desk phone, not to change the location of your office.
